This is a "Wire Line Modem"
This device is required to send secure data, such as DVP over twisted pair cables, or narrow mux channels.
This is full duplex, 2 channel, as it has four card sets.
It also has provisions for base station keying.
It uses QUAM modulation.
There has to be an identical modem on the other end.

This is T5091 12Kb Securenet Portable Modem in a Zero Case. It was most likely built for the Federal Government by the Florida SP Group. As Bernie has pointed out this set up is configured as Full Duplex so two pair of modem are configured for Transmit and the two pair are setup as Receive. You would need a pair of the boxes to compliment each in a system configuration, either using a RT3002 or T1 Telco lines to send and receive the QAM Data down the Telco lines providing the necessary secure communications signaling end-to-end throughout the entire system.
The T5007BX Console Interface Unit (CIU) perform the actual encryption / decryption for the Analog Console. The T5007BX is connected to the T5091A Securenet Modem, Telco Lines, T5091A Securenet Modem to a Securenet Micor, MSF5000 or Quantar with the appropriate set of options on the Base Station / Repeater. This unit can handle DVP, DVP-XL, DES, DES-XL and DVI-XL Formats.
This equipment has been Obsoleted from the Securenet Product Group since the year 2000.
