The first step would be to verify that the accy jack pads haven't been lifted off the PCB....that would necessitate replacing the board, i.e. back cover kit. Assuming that the seven mounting pads are still attached to the PCB, check for cracked solder joints at all pads as an aid to verifying that the problem is in the jack assembly itself. Assuming that the accy jack itself needs to be replaced, you'll need temperature-controlled soldering equipment, either hot air or a good soldering station and solder wick. Since the mounting pads are small, try to operate the soldering station at the lowest temperature possible, maybe around 640 deg fahrenheit for most of the pads to avoid detaching them from the board, and somewhat higher for the two pads that have larger traces under them. If you have hot air available, you can reflow all solder pads simultaneously and simply lift the jack assembly off the board. If you use a soldering station, you'll need a very small tip to heat the two lands nearest the center of the assembly(between the two jacks).

What is the model of the hot air equipment you use and what is the part number for the tip you use?
