Need a PCI/USB 2.0 card with an intenal 5pin header..

Ok, picked up one of those Dynex internal multicard readers. I thought I had a 5pin motheroboard spot for it, but turns out I dont.
So, since I could use some better USB2.0 support, I need an internal/USB PCI card with a 5pin header on it. I see all sorts of USB/PCI cards, none with headers (so far).
Anyone know of a make/model?
