Blank Punchblock Sheet

I have kind of a weird request.

I am looking for (and cant find anywhere) a word document, excell, or similiar file with a blank punch block on it. I have seen people document on these before by typing stuff instead of written, but I cant seem to find them anywhere...

I was wondering if maybe someone had something simliar so I could document a crapload of blocks I will be installing this week...

Something that looks like this, except editable so you can type into it?

yeah, i am looking for something SORTA like that...

I seen these before that actually look like a punch block and you could enter text in where the actual punches would be, and then the number to the side like it is on an actual block...

i know...sounds kinda stupid.....but i suck at explaining things...

anyways, whatever u come up with i am sure will be fine...shoot me an email with it :

My brain is soggy when it comes to punchdown blocks...

Standard telephone type for 25 pair (50 rows x 4 contacts across)?
