What is the model number of the Motorola or Amp crimping tool for the 10 pin accessory jack on the CDM/ CM series radios ?

If you are talking about the 20 pin accessory connector on the CDM series (not sure where the 10 pin is) then the AMP tool is # 91516-1. Its a lot of money for the tool - around $490. I just bought some leads with the pins pre crimped on one end with 4 connectors for $16 on "a famous online auction site...".

You might also want to try the Molex Crimp Service Tool #63811-1000. I bought one a few years ago for around $35 as I remember, but I note it now sells for $48 at Jameco. This tool will crimp a variety of Molex style connectors. It takes a little practice to find the right position for the crimp you need but good if your only doing a few.

Use a D-SUB crimp tool. They aren't that expensive and do a reasonably good job.
