Did you check to see if it worked before you programmed it?

Hi:

Well, I hate to be the bearer of bad news, but I would recommend taking the radio apart and see if perhaps it may have " Water Damage"...This can sometimes be seen if a " White" residue is left on the PC Board.

If your radio is " perfect" it should be sqeaky clean inside.

If its clean " Inside" there is a possibility
that the Code Plug may have been corrupted by
a Modification that went sour.

Depending on the radio, it may or may not be
worth while to attempt fixing it. Any Water
Damage seen inside, forget it...It will never
be reliable.

( I have seen lots of them )
